Terms for Applying For Certified School with Curriculum.

TERMS

Upon approval, you agree that all your students of Hypnosis shall enroll in a STUDENT membership with the International Medical & Dental Hypnotherapy Association. The student may upgrade their membership when they complete your training, pass the IMDHA Certification Assessment and pay a reduced fee within 6 months of Student membership.

School directors are required to provide IMDHA the equivalent of $250 in services annually. This revenue is generated through submission of IMDHA memberships. To fulfill your obligation, you may provide IMDHA with any of the following: 5 Student members, 2 Certified members, 3 Associate members or any combination thereof.

REQUIRED DOCUMENTATION With Application

  • Photo Id or Passport Photo
  • Copies of transcripts, certificates and documents to verify all information submitted in the completed registration form.
  • Payment in full of all fees in US Dollars by Check, Cashier's Check, Money Order or Major Credit Card.

All decisions are final. The IMDHA reserves the right to reject any application without justification. If for any reason an application is denied, payment is immediately refunded.
